This contract is between ADR Professional Investigations and _____________________________; herein known as the “Client”, who resides at_______________________________, telephone number ___________________, for ADR Professional Investigations to provide the following service or services.

ADR Professional Investigations is to provide ____________________________________________, for $ ____________ per hour, plus normal and expected expenses, with a retainer of $ ____________ to be paid by the client prior to the initiation of the services by ADR Professional Investigations. When the retainer is expended, additional funds must be provided by the Client to continue services. The additional funds to be provided will be calculated based on the time necessary to continue or complete the services and will be subject to the same provisions listed in this contract.

The Client will receive written reports detailing all services provided, days the services were provided and complete accounting of all funds received by ADR Professional Investigations, the remaining funds will be refunded to the Client.

A contract is required by the Arkansas State Board of Private Investigators, Private Security Agencies and Alarm System Companies, administered by the Special Services Division of the Arkansas State Police, 501-618-8600.
